Admission Procedures: The Department of Public Instruction has established the following procedures to be considered as an eligible choice student and qualify for a voucher.
Each student applicant must complete the MPCP Student Application and submit it to Rawhide Starr Academy Choice School during the following timeframes to be considered:
September 1 – September 14 November 1 – November 20 December 1 – January 7
The will also need to prove proof of residency of the student in the City of Milwaukee. There are no forms for proof of residency from the Department of Public Instruction.
The applicant must supply us with a current wage statement, utility bill, property tax bill or lease agreement (showing name of one of the parents/guardians at the address shown on the application). Rent receipts cannot be used for proof.
The Department of Public Instruction has specific forms that need to be completed in order to apply for the Milwaukee Parent Choice Program. Additional links are also provided to assist you in the application process. Please read through this entire page to ensure you complete all the requirements for your student.
All forms must be submitted to Starr Academy in person or via mail with original signatures. Other documents may be photocopied. We cannot accept emailed or faxed forms.
