Dental Insurance
Dental coverage plan through Lincoln Financial
Preventative checkups are covered at 100%
Deductible for basic and major dental work is $50/person/year, up to 3 deductibles per family
Basic dental work covered at 85%
Major dental work covered at 50%
Rawhide co-insures crowns (major dental) an additional 30%
Maximum benefit per person, per year is $1,000

Vision Insurance
Vision coverage through VIPA (Vision Insurance Plan of America)
Vision exam and lenses covered at 100% annually
Frames covered up to $125 retail value bi-annually
Contact lenses and exam covered up to $150 annually

Life Insurance
Benefit is two-and-a-half times annual salary, (based on a 40-hour week)
Benefit is reduced after age 64
No employee contribution for premiums
Disability Insurance
Benefit is 60% of normal wage (based on a 40-hour week)
Short-term benefits start after 10 consecutive workdays of disability
Long-term benefits start after 90 calendar days of disability
